No they don't all come with combs and brushes It completely depends on the release. Most of the earliest and a lot of the single releases do but as you progress through the line and the gimmicks and accompanying accessories change so do whether or not there is a comb or brush boxed as well. As to the explanation its probably as boring as cost. If they are boxing a pony with an accessory or accessories or with a gimmick or perhaps as an exclusive item, then there is no need to spend extra on the item to include a comb and brush. If the pony is just in a box by herself then they will probably pop one in as its a cheap way to bulk the product.
It could be both but its most likely that there was no comb/brush to begin with. Normally they are either front and centre so easy to spot or can at least be spotted from a quick inspection. They only possibility is if they come loose from there holding and they could then fall into hiding.

I think thats a safe assumption And if you are not certain you can always ask in the Corral and someone will be able to tell you for certain!
LOL - thats a perfect description! On occasion you may find a leaflet in the bottom of the box or just some general marketing nonsense but for these girls no comb and no brush and more often than not nothing at all.
