Why do some people get their own "Happy Birthday" topic and others not?

Some people start their own and some people have friends that start one for them.  If you don't fall into one of those categories then you don't get a post.

Yup, that's it pretty much. If someone knows it's your birthday and starts a thread, then it'll be there. If someone else hasn't started one, you can take the initiative to start your own. All birthday threads get stickied for a few days.

This is pretty unnecessary, there's a calendar with all the birthdays on it in the top menu

Just wanted to point out that not everyone's birthdays are in the calendar - only if someone chooses to include their birthday in their profile.
