Pretty Parlor Name Tags

Would anyone know a website that has clear scans of the pretty parlor name tags?
Or maybe a really kind person can provide a scan?
I got HavAcrumpet452 a parlor w/no accessories for her b-day and was hoping to print her the US (and maybe if someone has, the UK tags-which would be awesome) ones on some cardstock.
